The best supported treatment for tinnitus is a type of counseling called cognitive behavioral therapy (CBT) which can be delivered via the internet or in person.[5][77] It decreases the amount of stress those with tinnitus feel.[78] These benefits appear to be independent of any effect on depression or anxiety in an individual.[77] Acceptance and commitment therapy (ACT) also shows promise in the treatment of tinnitus.[79] Relaxation techniques may also be useful.[3] A clinical protocol called Progressive Tinnitus Management for treatment of tinnitus has been developed by the United States Department of Veterans Affairs.[80]
The cause of tinnitus may be difficult to determine. Your doctor will ask if you have been exposed to loud noise at work or home and will ask about medications you take, including all herbs and supplements. He or she may look in your ears to see if you have wax blockage or if the eardrum appears abnormal. If your hearing is affected, then your doctor may have you undergo a hearing test called an audiogram to measure your hearing ability in each ear.
Tinnitus (pronounced tin-NY-tus or TIN-u-tus) is not a disease. It is a symptom that something is wrong in the auditory system, which includes the ear, the auditory nerve that connects the inner ear to the brain, and the parts of the brain that process sound. Something as simple as a piece of earwax blocking the ear canal can cause tinnitus. But it can also be the result of a number of health conditions, such as:

In the advance online edition of Nature on January 12, 2011, the researchers reported that the number of neurons tuned to the high frequency had jumped by 79% compared to control rats. The scientist then tested 2 different tones in a second group of rats but stimulated the vagus nerve only for the higher one. The neurons tuned to the higher tone increased by 70%, while those tuned to the lower one decreased in number. This showed that the tone alone wasn’t enough to initiate the change; it had to be accompanied by VNS.

For many, tinnitus symptoms come on gradually and eventually go away as the brain and ears adjust. However, for others tinnitus can last for years and cause various complications. A high percentage of people with tinnitus that’s persistent and untreatable go on to also develop anxiety or depression as a result. What types of things can you do to deal with and lower tinnitus symptoms? Tinnitus treatment includes avoiding excessively loud sources of noise pollution, using certain hearing aids, preventing ear infections and avoiding drug use.

Smoking. Contrary to popular belief, there are some external irritants that can cause tinnitus. For example, Nicotine has been proven to be an irritant that can cause someone to develop a ringing in their ears. Smokers may find that their chances of developing the condition may be higher than someone who is a non-smoker. If you’re suffering from tinnitus right now, and you’re a smoker, please quit as soon as possible. If that’s just not an option for you right now, be sure to at least pick up an over the counter tinnitus treatment that will dramatically reduce the ringing in your ears.
Schecklmann et al (2014) suggested that tinnitus is associated with alterations in motor cortex excitability, by pooling several studies, and reported that there are differences in intracortical inhibition, intra-cortical facilitation, and cortical silent period. We doubt that this means that motor cortex excitability causes tinnitus, but rather we suspect that these findings reflect features of brain organization that may predispose certain persons to develop tinnitus over someone else.

Cochlear implants are sometimes used in people who have tinnitus along with severe hearing loss. A cochlear implant bypasses the damaged portion of the inner ear and sends electrical signals that directly stimulate the auditory nerve. Most people who seek medical help for tinnitus experience it as subjective, constant sound like constant ringing in the ears or a buzzing sound in the ear, and most have some degree of hearing loss. Things that cause hearing loss (and tinnitus) include loud noise, medications that damage the nerves in the ear (ototoxic drugs), impacted earwax, middle ear problems (such as infections and vascular tumors), and aging. Tinnitus can also be a symptom of Meniere's disease, a disorder of the balance mechanism in the inner ear.
Until recently, most tinnitus patients had little reason to believe doctors would ever be able to completely cure or reverse the affliction. Drug therapies had consistently failed, and so had more invasive procedures — including some surgeries to remove the auditory nerve that transmits sound from the ear to the brain, according to past research. Sound waves travel through the ear canal to the middle and inner ear, where hair cells in part of the cochlea help transform sound waves into electrical signals that then travel to the brain's auditory cortex via the auditory nerve. When hair cells are damaged — by loud noise or ototoxic drugs, for example — the circuits in the brain don't receive the signals they're expecting. This stimulates abnormal activity in the neurons, which results in the illusion of sound, or tinnitus.
Patulous Eustachian tubes can be associated with tinnitus. The Eustachian tube is a small canal that connects the middle ear to the back of the nose and upper throat. The Eustachian tube normally remains closed. In individuals with a patulous Eustachian tube, the tube is abnormally open. Consequently, talking, chewing, swallowing and other similar actions can cause vibrations directly onto the ear drum. For example, affected individuals may hear blowing sounds that are synchronized with breathing.
The treatment group (245 patients) received some elements of standard care (such as a masking device and hearing aid if needed), but also received CBT. The CBT included an extensive educational session, sessions with a clinical psychologist and group treatments involving “psychological education” explaining their condition, cognitive restructuring, exposure techniques, stress relief, applied relaxation and movement therapy.
An assessment of hyperacusis, a frequent accompaniment of tinnitus,[56] may also be made.[57] The measured parameter is Loudness Discomfort Level (LDL) in dB, the subjective level of acute discomfort at specified frequencies over the frequency range of hearing. This defines a dynamic range between the hearing threshold at that frequency and the loudnes discomfort level. A compressed dynamic range over a particular frequency range is associated with subjectve hyperacusis. Normal hearing threshold is generally defined as 0–20 decibels (dB). Normal loudness discomfort levels are 85–90+ dB, with some authorities citing 100 dB. A dynamic range of 55 dB or less is indicative of hyperacusis.[58][59]

Children with hearing loss have a high incidence of tinnitus, even though they do not express the condition or its effect on their lives.[100] Children do not generally report tinnitus spontaneously and their complaints may not be taken seriously.[101] Among those children who do complain of tinnitus, there is an increased likelihood of associated otological or neurological pathology such as migraine, juvenile Meniere’s disease or chronic suppurative otitis media.[102] Its reported prevalence varies from 12% to 36% in children with normal hearing thresholds and up to 66% in children with a hearing loss and approximately 3–10% of children have been reported to be troubled by tinnitus.[103]
The researchers next tested whether tinnitus could be reversed in noise-exposed rats. The animals received VNS paired with various tones other than the tinnitus frequency 300 times a day for about 3 weeks. Rats that received the treatment showed behavioral changes indicating that the ringing had stopped. Neural responses in the brain's auditory cortex returned to their normal levels as well, indicating that the tinnitus had disappeared.
Additional conditions that can cause pulsatile tinnitus include arterial bruit, abnormal passages or connections between the blood vessels of the outermost layer of the membrane (dura) that covers the brain and spinal cord (dural arteriovenous shunts), or conditions that cause increased pressure within the skull such as idiopathic intracranial hypertension (pseudotumor cerebri). Sigmoid sinus dehiscence may be associated with pseudotumor, but this connection has not been firmly established. It possible that cases of pulsatile tinnitus associated with pseudotumor may be caused by an undiagnosed SSWA. Head trauma, surgery, middle ear conductive hearing loss, and certain tumors can also cause pulsatile tinnitus. Obstructions within in the vessels that connect the heart and brain can also cause pulsatile tinnitus.

Masking Devices. Similar to the white noise machines listed above, there are now masking devices that can be worn in the ear, just like a hearing aid, that do almost the same thing. They produce low-level white noise that can suppresses your tinnitus symptoms by training your brain to focus on them instead of the ringing in your ears. These are perfect if you can’t always have a white noise machine running near you.
Lidocaine, a medication used for the treatment of certain types of abnormal heart rhythms, has been shown to relieve tinnitus for some people, but it must be given intravenously or into the middle ear to be effective. However, the benefits of lidocaine are almost always outweighed by the risks of the drug and it is therefore not recommended and not used for tinnitus.
