Subjective tinnitus is the most frequent type of tinnitus. It can have many possible causes, but most commonly it results from hearing loss. When the tinnitus is caused by disorders of the inner ear or auditory nerve it is called otic (from the Greek word for ear). These otological or neurological conditions include those triggered by infections or drugs. A frequent cause is noise exposure that damages hair cells in the inner ear.
Tinnitus is a non-curable, invisible and debilitating hearing disorder that can take on many different forms – ringing, hissing, buzzing, and even the sound of crickets. Almost everyone has experienced brief periods of mild tinnitus, but for many, this sound can be permanent. Over 360,000 Canadians report suffering from chronic tinnitus, and almost half of those are severely affected.1 In the US, over 16 million tinnitus sufferers seek treatment every year.2 Tinnitus is the number one disability claim for US veterans3 and has also become the top disability claim for current and former male RCMP members.4 This persistent sound can have a serious impact on quality of life; leading to sleep deprivation, depression, anxiety, and even suicide. What adds to the challenges faced by tinnitus sufferers is a lack of knowledge, support and options available to them. Unfortunately, there are currently too few health care professionals providing services to tinnitus sufferers who are seeking ways to manage their tinnitus. Unfortunately, the phrase “learn to live with it” is still heard far too often by those that seek help for tinnitus.
Tinnitus Retraining Therapy. Tinnitus Retraining Therapy (TRT) combines a wearable device that is individually programmed to mask the specific tonal frequency of that person’s tinnitus, with psychological therapy that teaches a patient to ignore the sounds his tinnitus is creating. I consider it the best of all of the above noise suppression techniques, as it is individually tailored for each person and involves support from a trained psychological therapist. It is also the most expensive and time consuming, but in my medical opinion, the most beneficial of all the noise suppression techniques listed above.
Standard masking devices help to mask the sound of tinnitus while you are using them, but they have no long-lasting effects. Modern medical-grade devices use customized sounds tailored specifically to your tinnitus. Unlike regular sound machines, these devices are only worn intermittently. You may experience benefits long after the device is turned off, and over time, you may experience long-term improvement in the perceived loudness of your tinnitus.
A disorder of the inner ear, Ménière’s disease typically affects hearing and balance and may cause debilitating vertigo, hearing loss, and tinnitus. People who suffer from Ménière’s disease often report a feeling of fullness or pressure in the ear (it typically affects only one ear). The condition most often impacts people in their 40s and 50s, but it can afflict people of all ages, including children. Although treatments can relieve the symptoms of Ménière’s disease and minimize its long-term influence, it is a chronic condition with no true cure.

Changes in the bones of the middle ear. A person’s ear is made up of several different bones: the malleus, Incus and Stapes. In some individuals, these bones may actually change shape or harden over the years. This process is known as otosclerosis and often runs in the family. This can cause ringing in the ears to begin or, if it has already started, to get worse over time.

The exact biological process by which hearing loss is associated with tinnitus is still being investigated by researchers. However, we do know that the loss of certain sound frequencies leads to specific changes in how the brain processes sound. In short, as the brain receives less external stimuli around a specific frequency, it begins to adapt and change. Tinnitus may be the brain’s way of filling in the missing sound frequencies it no longer receives from the auditory system.
The physician may also request an OAE test (which is very sensitive to noise induced hearing damage), an ECochG (looking for Meniere's disease and hydrops, an MRI/MRA test (scan of the brain), a VEMP (looking for damage to other parts of the ear) and several blood tests (ANA, B12, FTA, ESR, SMA-24, HBA-IC, fasting glucose, TSH, anti-microsomal antibodies).
Earwax (ear wax) is a natural substance secreted by special glands in the skin on the outer part of the ear canal. It repels water, and traps dust and sand particles. Usually a small amount of wax accumulates, dries up, and then falls out of the ear canal carrying with it unwanted particles. Under ideal circumstances, you should never have to clean your ear canals. The absence of ear wax may result in dry, itchy ears, and even infection. Ear wax may accumulate in the ear for a variety of reasons including; narrowing of the ear canal, production of less ear wax due to aging, or an overproduction of ear wax in response to trauma or blockage within the ear canal.
Spontaneous otoacoustic emissions (SOAEs), which are faint high-frequency tones that are produced in the inner ear and can be measured in the ear canal with a sensitive microphone, may also cause tinnitus. About 8% of those with SOAEs and tinnitus have SOAE-linked tinnitus,[need quotation to verify] while the percentage of all cases of tinnitus caused by SOAEs is estimated at about 4%.
No matter what the cause, the condition interrupts the transmission of sound from the ear to the brain. Some of the neural circuits no longer receive signals. Strangely, this does not cause hearing loss. Instead, when neural circuits don’t receive stimulation, they react by chattering together, alone at first and then synchronous with each other. Once the nerve cells become hyperactive and occur at the same time, they simulate a tone the brain “hears” as tinnitus. Analogous to a piano, the broken “keys” create a permanent tone without a pianist playing the keys.
Atherosclerosis. With age and buildup of cholesterol and other deposits, major blood vessels close to your middle and inner ear lose some of their elasticity — the ability to flex or expand slightly with each heartbeat. That causes blood flow to become more forceful, making it easier for your ear to detect the beats. You can generally hear this type of tinnitus in both ears.
