Tinnitus can be perceived in one or both ears or in the head. It is the description of a noise inside a person’s head in the absence of auditory stimulation. The noise can be described in many different ways. It is usually described as a ringing noise but, in some patients, it takes the form of a high-pitched whining, electric buzzing, hissing, humming, tinging or whistling sound or as ticking, clicking, roaring, "crickets" or "tree frogs" or "locusts (cicadas)", tunes, songs, beeping, sizzling, sounds that slightly resemble human voices or even a pure steady tone like that heard during a hearing test. It has also been described as a "whooshing" sound because of acute muscle spasms, as of wind or waves.[not in citation given] Tinnitus can be intermittent or continuous: in the latter case, it can be the cause of great distress. In some individuals, the intensity can be changed by shoulder, head, tongue, jaw or eye movements. Most people with tinnitus have some degree of hearing loss.
Tinnitus also could be the result of neural circuits thrown out of balance when damage in the inner ear changes signaling activity in the auditory cortex, the part of the brain that processes sound. Or it could be the result of abnormal interactions between neural circuits. The neural circuits involved in hearing aren’t solely dedicated to processing sound. They also communicate with other parts of the brain, such as the limbic region, which regulates mood and emotion.
Most people who suffer from tinnitus also experience hearing loss to some degree. As they often accompany one another, the two conditions may be correlated. In fact, some researchers believe that subjective tinnitus can only occur if the auditory system has been previously damaged (source). The loss of certain sound frequencies due to hearing loss may change how the brain processes sound, causing it to adapt and fill in the gaps with tinnitus. Auditory-somatosensory stimulation is a similar treatment approach to Kilgard’s, in that its goal is to retune the faulty patterns of brain activity that can cause tinnitus. It involves pairing sounds played in the ear with specially timed electric impulses, which are administered to touch-sensitive nerves using a pad attached to the neck, Dr. Shore explains about the research she’s working on.
The treatment group (245 patients) received some elements of standard care (such as a masking device and hearing aid if needed), but also received CBT. The CBT included an extensive educational session, sessions with a clinical psychologist and group treatments involving “psychological education” explaining their condition, cognitive restructuring, exposure techniques, stress relief, applied relaxation and movement therapy.
This well-designed study found that using CBT alongside elements of standard therapy can help patients with tinnitus of varying severity. However, the differences in outcomes between the two groups were quite small, and this technique can only help manage tinnitus rather than curing it, as some papers implied. Also, the patients in the study were followed for only 12 months, so it is unclear whether this approach can help in the longer term.
A disorder of the inner ear, Ménière’s disease typically affects hearing and balance and may cause debilitating vertigo, hearing loss, and tinnitus. People who suffer from Ménière’s disease often report a feeling of fullness or pressure in the ear (it typically affects only one ear). The condition most often impacts people in their 40s and 50s, but it can afflict people of all ages, including children. Although treatments can relieve the symptoms of Ménière’s disease and minimize its long-term influence, it is a chronic condition with no true cure.

For some people, the jarring motion of brisk walking can produce what is called a seismic effect which causes movement in the small bones or contractions in the muscles of the middle ear space. You can experiment to find out if this is the cause by walking slowly and smoothly to see if the clicking is present. Then, try walking quickly and with a lot of motion to see if you hear the clicking. You can also test for the seismic effect by moving your head up and down quickly.
