Traumatic brain injury, caused by concussive shock, can damage the brain’s auditory processing areas and generate tinnitus symptoms. TBI is one of the major catalysts for tinnitus in military and veteran populations. Nearly 60% of all tinnitus cases diagnosed by the U.S. Veterans Administration are attributable to mild-to-severe traumatic brain injuries.
But it’s still a significant improvement. And Kilgard says he and others are working to make the treatment even more effective. He suspects this type of therapy is not too far off from being available to patients outside of research studies. “It’s in the late stages of development,” he says. “It could be available to the public in as little as a year or two.”

There are many different conditions and disorders that affect nerve channels leading to the ears, which can cause someone to hear abnormal ringing or other sounds in their ears. These conditions usually cause other symptoms at the same time (such as dizziness, hearing loss, headaches, facial paralysis, nausea and loss of balance), which doctors use as clues to uncover the underlying cause of tinnitus.
To understand what causes tinnitus, you first need to understand what tinnitus is. Tinnitus is, very simply, unexplainable noises you hear in your head when there is no actual sound present. A person with tinnitus will often hear a whistling, humming, buzzing, whooshing, clicking or ringing in their ears, even when there is nothing in the area that is emitting that particular sound. It may be intermittent or last only a short time or never seem to stop.
Temporomandibular joint (TMJ) syndrome is a disorder that causes symptoms like pain, clicking, and popping of the jaw. TMJ is caused by injury to the temporomandibular joint. Stress, poor posture, jaw trauma, genetic predisposition, and inflammatory disorders are risk factors for the condition. A variety of self-care measures (application of ice, use of over-the-counter pain medication, massage, relaxation techniques) and medical treatment options (dental splint, Botox, prescription medications, surgery) are available to manage TMJ. The prognosis of TMJ is good with proper treatment.
Other causes of tinnitus include middle ear infections, disorders that block the ear canal (such as an external ear infection [external otitis], excessive ear wax, or foreign bodies), problems with the eustachian tube (which connects the middle ear and the back of the nose) due to allergies or other causes of obstruction, otosclerosis (a disorder of excess bone growth in the middle ear), and temporomandibular disorders. An uncommon but serious cause is an acoustic neuroma, a noncancerous (benign) tumor of part of the nerve leading from the inner ear.
What does he mean by “ends up in the brain”? Essentially, something that causes even temporary hearing damage — such as exposure to very loud noise or a blow to the head — can change activity patterns in the brain in ways that cause the ringing. Even though some damage or problem in the ear triggered tinnitus to begin with, you continue to hear the sound you do because of a signal from the brain.
To keep the brain activated and aware, Kilgard’s therapy involves stimulating the vagus nerve, which is actually a pair of nerves that runs inside the neck and into the brain. “All the stuff you brains learns about your body — it all comes in through the vagus nerve,” he says. “We trick the brain into thinking it’s learning something important by stimulating this nerve in the neck.”

Some patients question the value of treatments that fall short of an absolute cure. ATA believes patients should do everything possible to lessen the burden of tinnitus until a definitive cure is found. An appropriate analogy may be the use of ibuprofen for a headache. Ibuprofen itself does not cure the underlying cause of most headaches, but it does reduce the pain that makes headaches feel so awful. Likewise, the most effective tinnitus treatment tools address the aspects of tinnitus that so often make the condition feel burdensome: anxiety, stress, social isolation, sound sensitivity, hearing difficulties, and perceived volume.
When tinnitus is unexpected and unwelcomed, it can lead to a negative reaction to the tinnitus. This can create a vicious cycle. When tinnitus is perceived, it can prompt emotions, including frustration, fear, unhappiness, etc. These can, in turn, cause physical reactions such as anxiety and stress. This reinforces the tinnitus and perpetuates the cycle.
The multidisciplinary approach required input from many different professionals including audiologists, psychologists, speech therapists and physical therapists. Which particular care elements of the intervention had the greatest effect is unknown. A multidisciplinary approach such as the intervention trialled here may have resource implications if it were introduced into standard clinical practice.

Tinnitus is a condition in which you hear noises when there is no outside source of the sounds. The noises can have many different forms (ringing, clicking, buzzing, roaring, etc.) and can be soft or loud. Treatment options include hearing aids; tinnitus masking devices; devices that increase background noise levels; coping, relaxation, anxiety control methods; and counseling and retraining therapy.Tinnitus does not typically occur in children.
In addition, a healthy lifestyle can reduce the impact of tinnitus. Avoid physical and emotional stress, as these can cause or intensify tinnitus. You may be able to reduce your stress levels through exercise, meditation, deep breathing, or massage therapy. If you suffer from high blood pressure, consult your doctor for help controlling it, as this can also impact tinnitus. Finally, get plenty of rest to avoid fatigue and exercise regularly to improve your circulation. Although this won’t eliminate the ringing in your ears, it may prevent it from worsening.
Vascular issues. Some people have blood vessels near their ears that are capable of causing tinnitus. I have found that if the blood pressure is elevated, this increased pressure can cause that dreaded ringing in your ears or even a whooshing sound. Because pregnant women often have elevated blood pressure, they are easily susceptible to tinnitus. Tinnitus caused by pregnancy should go away with an over the counter tinnitus treatment and once the baby is born. An overactive thyroid has also been shown to causes vascular issues that bring on tinnitus.
Almost every ENT, audiology practice, and hearing aid dispenser who claims to offer tinnitus treatment only offers one solution: hearing aids. While amplification may help some, only 50% of people living with tinnitus experience hearing loss that affects their understanding of speech, which means hearing aids are ineffective. A loud work environment. A loud work environment that involves the use of power tools, power saws, drills or other noisy equipment may cause temporary bouts of tinnitus. I know of many tinnitus suffers who have attended rock concerts and left with ringing in their ears that may take hours or even days to subside. The longer a person remains in that loud environment, the better their chances will be of developing the condition permanently. These environments can also cause hearing loss. Always wear earplugs when you are in a loud environment, even if it is only going to be for a short time. Mowing the lawn? Wear earplugs.
But one of the awesome powers of the human brain is its adaptability. “It can learn and reorganize itself every time you practice something new,” Kilgard says. His research, including a study published in February 2014 in the journal Neuromodulation, has shown this adaptability may be key to helping the brain “turn down” the hyperactivity that can lead to tinnitus, he says.
