Tinnitus retraining therapy (TRT). This technique is based on the assumption that tinnitus results from abnormal neuronal activity (see "What's going on?"). The aim is to habituate the auditory system to the tinnitus signals, making them less noticeable or less bothersome. The main components of TRT are individual counseling (to explain the auditory system, how tinnitus develops, and how TRT can help) and sound therapy. A device is inserted in the ear to generate low-level noise and environmental sounds that match the pitch, volume, and quality of the patient's tinnitus. Depending on the severity of the symptoms, treatment may last one to two years.

Noise exposure. Exposure to loud noises can damage the outer hair cells, which are part of the inner ear. These hair cells do not grow back once they are damaged. Even short exposure to very loud sounds, such as gunfire, can be damaging to the ears and cause permanent hearing loss. Long periods of exposure to moderately loud sounds, such as factory noise or music played through earphones, can result in just as much damage to the inner ear, with permanent hearing loss and tinnitus. Listening to moderately loud sounds for hours at a young age carries a high risk of developing hearing loss and tinnitus later in life.
For many, tinnitus symptoms come on gradually and eventually go away as the brain and ears adjust. However, for others tinnitus can last for years and cause various complications. A high percentage of people with tinnitus that’s persistent and untreatable go on to also develop anxiety or depression as a result. What types of things can you do to deal with and lower tinnitus symptoms? Tinnitus treatment includes avoiding excessively loud sources of noise pollution, using certain hearing aids, preventing ear infections and avoiding drug use.
ABR (ABR) testing may show some subtle abnormalities in otherwise normal persons with tinnitus (Kehrle et al, 2008). The main use of ABR (ABR test) is to assist in diagnosing tinnitus due to a tumor of the 8th nerve or tinnitus due to a central process. A brain MRI is used for the same general purpose and covers far more territory, but is roughly 3 times more expensive. ABRs are generally not different between patients with tinnitus with or without hyperacusis (Shim et al, 2017).
Changes in the bones of the middle ear. A person’s ear is made up of several different bones: the malleus, Incus and Stapes. In some individuals, these bones may actually change shape or harden over the years. This process is known as otosclerosis and often runs in the family. This can cause ringing in the ears to begin or, if it has already started, to get worse over time.

People with tinnitus actually do "hear" noises that range from a whistle to a crackling noise to a roar. It can happen only occasionally, can occur for a period of days then take a break before recurring again, or it can be constant. The sound can vary in pitch from quiet to unbearably loud, or it can stay the same.
There is a growing body of evidence suggesting that some tinnitus is a consequence of neuroplastic alterations in the central auditory pathway. These alterations are assumed to result from a disturbed sensory input, caused by hearing loss.[28] Hearing loss could indeed cause a homeostatic response of neurons in the central auditory system, and therefore cause tinnitus.[29]

According to ATA, The American Tinnitus Association, 45 million Americans are struggling with tinnitus. Tinnitus is not a disease in and of itself, but rather a symptom of some other underlying health condition. In most cases, tinnitus is a sensorineural reaction in the brain to damage in the ear and auditory system. While tinnitus is often associated with hearing loss, there are roughly 200 different health disorders that can generate tinnitus as a symptom. Below is a list of some of the most commonly reported catalysts for tinnitus.


Seek treatment for hearing problems. If you’re experiencing difficulty hearing, talk to your physician and seek help from an otolaryngologist (an ear, nose throat specialist) or an audiologist. In addition to addressing any underlying health issue and improving your quality of life, improving your hearing can make tinnitus less noticeable and less bothersome, during the day and at night when you’re trying to sleep.
It is important to note that existing hearing loss is sometimes not directly observable by the patient, who may not perceive any lost frequencies. But this this does not mean that hearing damage has not been done. A trained audiologist or other hearing health professional can perform sensitive audiometric tests to precisely measure the true extent of hearing loss.

Cognitive behavioral therapy (CBT). CBT uses techniques such as cognitive restructuring and relaxation to change the way patients think about and respond to tinnitus. Patients usually keep a diary and perform "homework" to help build their coping skills. Therapy is generally short-term — for example, weekly sessions for two to six months. A 2010 review of six studies by the Cochrane Collaboration (an international group of health authorities who evaluate randomized trials) found that after CBT, the sound was no less loud, but it was significantly less bothersome, and patients' quality of life improved.

Many people find that tinnitus causes frustration, stress, and even anger. And unfortunately, your exasperation and anxiety can seem to amplify the issue. Learning how to thoroughly relax can help you manage your tinnitus. Deep breathing, meditation, yoga, or music therapy may help in combination with sound therapy. You could also explore relaxing hobbies like gardening, painting, swimming, photography, knitting, reading, cooking, or other physical activities (walking, biking, etc.).
One of the big problems associated with curing tinnitus, experts say, is that it’s really a symptom of multiple conditions, as opposed to being a single condition with a predictable trigger. In fact, more than 200 different conditions — problems ranging from hearing loss to head or neck trauma — have been linked with tinnitus, which makes it a real bear to try to stop. (3)

Exposure to Loud Noise: Exposure to loud or excessive noise can damage or destroy hair cells (cilia) in the inner ear. Because the hair cells cannot be renewed or replaced, this can lead to permanent hearing loss and/or tinnitus. Continued exposure can worsen these conditions, so people who work in loud environments should always wear ear protection. This includes musicians, air traffic controllers, construction workers, military personnel, and first responders. In addition, consider lowering the volume on your iPod and wearing earplugs at loud concerts.
Hyperacusis is a different, but related condition to tinnitus. People with hyperacusis have a high sensitivity to common, everyday environmental noise. In particular, sharp and high-pitched sounds are very difficult for people with hyperacusis to tolerate—sounds like the screeching of brakes, a baby crying or a dog barking, a sink full of dishes and silverware clanging.  Many people with tinnitus also experience hyperacusis—but the two conditions don’t always go together.
Although drugs cannot cure tinnitus, there are a few that will help suppress the symptoms you are experiencing. Tricyclic antidepressants, like amitriptyline and nortriptyline, are two of the most commonly prescribed medications. If you are experiencing severe tinnitus, one of these drugs may be used. However, it's important to know that these medications may come with side effects such as dry mouth, blurry vision and heart issues. Discuss any other conditions you have or medications you are currently taking with your physician. Niravam and Xanax can also be prescribed, but each of these medications can cause drowsiness and nausea, and they can be habit-forming.
Before long, you’re both mentally and physically stimulated in ways that make it even harder to relax and fall asleep. Like any other form of anxiety, stress about falling asleep creates mental arousal, bringing your brain to alertness. And it also creates physical arousal, raising heart rate and body temperature. This kind of anxiety can lead to behaviors that further undermine sleep, including:
Most tinnitus is "sensorineural," meaning that it's due to hearing loss at the cochlea or cochlear nerve level. But tinnitus may originate in other places. Our bodies normally produce sounds (called somatic sounds) that we usually don't notice because we are listening to external sounds. Anything that blocks normal hearing can bring somatic sounds to our attention. For example, you may get head noise when earwax blocks the outer ear.
