While there may be a wide range of causes, an important underlying factor for the development of tinnitus is brain plasticity.5,7 This property allows the brain to change and adapt, and it is essential to how we learn. Unfortunately, in some cases, such as with hearing loss, the auditory part of the brain may be altered as brain plasticity tries to compensate for the abnormal auditory inputs. This response leads to changes in brain activity in the auditory system (e.g., the auditory cortex) that can create a phantom percept: tinnitus. As such, while tinnitus may begin a problem at the auditory periphery, it persists because of changes throughout the auditory system. Treating tinnitus may require addressing both the initiator (e.g., hearing loss) and the driver (changes in the auditory brain).
Tinnitus is commonly thought of as a symptom of adulthood, and is often overlooked in children. Children with hearing loss have a high incidence of tinnitus, even though they do not express the condition or its effect on their lives. Children do not generally report tinnitus spontaneously and their complaints may not be taken seriously. Among those children who do complain of tinnitus, there is an increased likelihood of associated otological or neurological pathology such as migraine, juvenile Meniere’s disease or chronic suppurative otitis media. Its reported prevalence varies from 12% to 36% in children with normal hearing thresholds and up to 66% in children with a hearing loss and approximately 3–10% of children have been reported to be troubled by tinnitus.
There are two types of tinnitus: subjective tinnitus and objective tinnitus. Tinnitus is usually subjective, meaning that there is no sound detectable by other means. Subjective tinnitus has also been called "tinnitus aurium", "non-auditory" or "non-vibratory" tinnitus. In very rare cases tinnitus can be heard by someone else using a stethoscope, and in less rare – but still uncommon – cases it can be measured as a spontaneous otoacoustic emission (SOAE) in the ear canal. In such cases it is objective tinnitus, also called "pseudo-tinnitus" or "vibratory" tinnitus.
Tinnitus (pronounced "tin-it-tus") is an abnormal noise in the ear (note that it is not an "itis" -- which means inflammation). Tinnitus is common -- nearly 36 million Americans have constant tinnitus and more than half of the normal population has intermittent tinnitus. Another way to summarize this is that about 10-15% of the entire population has some type of constant tinnitus, and about 20% of these people (i.e. about 1% of the population) seek medical attention (Adjamian et al, 2009). Similar statistics are found in England (Dawes et al, 2014) and Korea (Park and Moon, 2014).
A number of vital tasks carried out during sleep help maintain good health and enable people to function at their best. Sleep needs vary from individual to individual and change throughout your life. The National Institutes of Health recommend about 7-9 hours of sleep each night for older, school-aged children, teens, and most average adults; 10-12 for preschool-aged children; and 16-18 hours for newborns. Cognitive Behavior Therapy (CBT) / Hypnotherapy. Another alternative treatment option worth considering is Cognitive Behavior Therapy (CBT), which is often simply referred to as hypnotherapy. The goal of CBT is to help lower any anxiety, anger or depression you are feeling toward your tinnitus, as well as to help retrain your brain to notice the ringing in your ears less. CBT is typically used in conjunction with sound stimulation therapies, like Neuromonics or Transcranial Magnetic Stimulation (TRT). When choosing your therapist, be sure they not only do they have previous experience working with patients with tinnitus, but also have Neural Linguistic Programming (NLP) training.
Even with all of these associated conditions and causes, some people develop tinnitus for no obvious reason. Most of the time, tinnitus isn’t a sign of a serious health problem, although if it’s loud or doesn’t go away, it can cause fatigue, depression, anxiety, and problems with memory and concentration. For some, tinnitus can be a source of real mental and emotional anguish.
Practice mindfulness meditation. I’ve written about the power of mindfulness mediation to reduce stress and improve sleep. A 2017 study found mindfulness meditation is also effective in helping people better manage tinnitus. Mindfulness meditation involves sitting comfortably, putting your attention on your natural breathing. When your mind wanders—to irritating tinnitus sounds, to worry about sleep, or wherever else it goes, gently return your attention to your breath. Start with a 5-minute session, and as you grow more comfortable with the practice, you can increase the time. You can practice mindfulness meditation anywhere, at any time of day—including in the shower!
Shore says her therapy isn’t for everyone — at least not yet. So far, she’s only treated patients who have a specific form of tinnitus that changes in intensity or pitch when a person moves certain parts of her body. For example, some tinnitus sufferers find the sound in their ears lessens when they clench their teeth or open their mouths wide. This suggests that some touch inputs can influence the tinnitus, Shore says. (Roughly two-thirds of tinnitus patients have this form of the condition, she adds.)
Repetitive transcranial magnetic stimulation (rTMS). This technique, which uses a small device placed on the scalp to generate short magnetic pulses, is already being used to normalize electrical activity in the brains of people with epilepsy. Preliminary trials of rTMS in humans, funded by the NIDCD, are helping researchers pinpoint the best places in the brain to stimulate in order to suppress tinnitus. Researchers are also looking for ways to identify which people are most likely to respond well to stimulation devices.
The diagnosis of tinnitus is usually based on the person's description. A number of questionnaires exist that may help to assess how much tinnitus is interfering with a person's life. The diagnosis is commonly supported by an audiogram and a neurological examination. If certain problems are found, medical imaging, such as with MRI, may be performed. Other tests are suitable when tinnitus occurs with the same rhythm as the heartbeat. Rarely, the sound may be heard by someone else using a stethoscope, in which case it is known as objective tinnitus. Spontaneous otoacoustic emissions, which are sounds produced normally by the inner ear, may also occasionally result in tinnitus.
Almost every ENT, audiology practice, and hearing aid dispenser who claims to offer tinnitus treatment only offers one solution: hearing aids. While amplification may help some, only 50% of people living with tinnitus experience hearing loss that affects their understanding of speech, which means hearing aids are ineffective. Tinnitus matching is helpful to identify the frequency and intensity of the tinnitus. This is a simple procedure in which the audiologist adjusts a sound until a patient indicates that it is the same as their tinnitus. Most patients match their tinnitus to the region of their hearing loss (Konig et al, 2006; Mahboubi et al, 2012). Unfortunately, the "gap detection test", does not work to confirm tinnitus in humabs (Boyen et al, 2015).
Earwax (ear wax) is a natural substance secreted by special glands in the skin on the outer part of the ear canal. It repels water, and traps dust and sand particles. Usually a small amount of wax accumulates, dries up, and then falls out of the ear canal carrying with it unwanted particles. Under ideal circumstances, you should never have to clean your ear canals. The absence of ear wax may result in dry, itchy ears, and even infection. Ear wax may accumulate in the ear for a variety of reasons including; narrowing of the ear canal, production of less ear wax due to aging, or an overproduction of ear wax in response to trauma or blockage within the ear canal.
Seek treatment for hearing problems. If you’re experiencing difficulty hearing, talk to your physician and seek help from an otolaryngologist (an ear, nose throat specialist) or an audiologist. In addition to addressing any underlying health issue and improving your quality of life, improving your hearing can make tinnitus less noticeable and less bothersome, during the day and at night when you’re trying to sleep.
Tinnitus is the name for hearing a sound that is not physically present in the environment. Some researchers have also described tinnitus as a “phantom auditory perception.” People with tinnitus most often describe it as ringing, buzzing, cricket sounds, humming, and whooshing, although many other descriptions have been used. In addition, a healthy lifestyle can reduce the impact of tinnitus. Avoid physical and emotional stress, as these can cause or intensify tinnitus. You may be able to reduce your stress levels through exercise, meditation, deep breathing, or massage therapy. If you suffer from high blood pressure, consult your doctor for help controlling it, as this can also impact tinnitus. Finally, get plenty of rest to avoid fatigue and exercise regularly to improve your circulation. Although this won’t eliminate the ringing in your ears, it may prevent it from worsening.
Inspection of the eardrum may sometimes demonstrate subtle movements due to contraction of the tensor tympani (Cohen and Perez, 2003). Tensor tympani myoclonus causes a thumping. Another muscle, the stapedius, can also make higher pitched sounds. See this page for more. Opening or closing of the eustachian tube causes a clicking. The best way to hear "objective tinnitus" from the middle ear is simply to have an examiner with normal hearing put their ear up next to the patient. Stethoscopes favor low frequency sounds and may not be very helpful.
Tinnitus also could be the result of neural circuits thrown out of balance when damage in the inner ear changes signaling activity in the auditory cortex, the part of the brain that processes sound. Or it could be the result of abnormal interactions between neural circuits. The neural circuits involved in hearing aren’t solely dedicated to processing sound. They also communicate with other parts of the brain, such as the limbic region, which regulates mood and emotion.
Tinnitus (pronounced tin-NY-tus or TIN-u-tus) is not a disease. It is a symptom that something is wrong in the auditory system, which includes the ear, the auditory nerve that connects the inner ear to the brain, and the parts of the brain that process sound. Tinnitus remains a symptom that affects the lives of millions of people. Research is directed not only at its treatment, but also at understanding why it occurs. Research by doctors at the University at Buffalo, The State University of New York, Dalhousie University (Canada), and Southeast China University have published research using electrophysiology and functional MRI to better understand what parts of the brain are involved in hearing and the production of tinnitus. Their research has found that much larger areas of the brain are involved with the process of hearing than previously believed, which may help direct future diagnostic and therapeutic options.
Prolonged exposure to loud sounds is the most common cause of tinnitus. Up to 90% of people with tinnitus have some level of noise-induced hearing loss. The noise causes permanent damage to the sound-sensitive cells of the cochlea, a spiral-shaped organ in the inner ear. Carpenters, pilots, rock musicians, street-repair workers, and landscapers are among those whose jobs put them at risk, as are people who work with chain saws, guns, or other loud devices or who repeatedly listen to loud music. A single exposure to a sudden extremely loud noise can also cause tinnitus.
