The multidisciplinary approach required input from many different professionals including audiologists, psychologists, speech therapists and physical therapists. Which particular care elements of the intervention had the greatest effect is unknown. A multidisciplinary approach such as the intervention trialled here may have resource implications if it were introduced into standard clinical practice.
Almost everyone has had tinnitus for a short time after being exposed to extremely loud noise. For example, attending a loud concert can trigger short-lived tinnitus. Some medications (especially aspirin and other nonsteroidal anti-inflammatory drugs taken in high doses) can cause tinnitus that goes away when the drug is discontinued. When it lasts more than six months, it's known as chronic tinnitus. As many as 50 to 60 million people in the United States suffer from this condition; it's especially common in people over age 55 and strongly associated with hearing loss. Many people worry that tinnitus is a sign that they are going deaf or have another serious medical problem, but it rarely is.
According to the American Tinnitus Association, this complex audiological and neurological condition is experienced by nearly 50 million Americans. (2) Older adults, men, people who smoke or use drugs, and those with a history of ear infections or cardiovascular disease have the highest risk for developing tinnitus. Most experts believe that it’s not a disorder itself, but rather one symptom of another underlying disorder that affects auditory sensations and nerves near the ears. However, there are tinnitus treatment options out there to treat those symptoms.
We are all different and you may find you prefer one type of relaxation over another. You may find a class that teaches a type you like. However, you may not be able to get to classes or you may just prefer to do something yourself. Using some simple techniques regularly may help you to improve your quality of life and make a real difference to living with tinnitus. It does take practice to develop good relaxation techniques, and what may help one day, may not do so the next – so don’t give up if at first it does not seem to help.
Hearing (audiological) exam. As part of the test, you'll sit in a soundproof room wearing earphones through which will be played specific sounds into one ear at a time. You'll indicate when you can hear the sound, and your results are compared with results considered normal for your age. This can help rule out or identify possible causes of tinnitus.
It’s the same mechanism that’s happening in people who feel a phantom limb sensation after losing a limb, explains Susan Shore, PhD, a professor of otolaryngology, molecular physiology, and biomedical engineering at the University of Michigan in Ann Arbor. With tinnitus the loss of hearing causes specific brain neurons to increase their activity as a way of compensating, she explains. “These neurons also synchronize their activity as they would if there were a sound there, but there is no external sound,” she adds.
A diagnosis of tinnitus is based upon identification of characteristic symptoms, a detailed patient history, a thorough clinical evaluation and complete audiologic testing. These steps will help to differentiate rhythmic tinnitus from non-rhythmic tinnitus. It cannot be overemphasized that tinnitus is a symptom of another underlying condition and not a diagnosis in and of itself. Because of the high number of underlying causes of tinnitus, a variety of specialized tests to detect the specific cause may be necessary. Attempting to identify the underlying cause of tinnitus is the first step in evaluating a person with tinnitus.
Resetting the tonotopic map. Researchers are exploring how to take advantage of the tonotopic map, which organizes neurons in the auditory cortex according to the frequency of the sound to which they respond. Previous research has shown a change in the organization of the tonotopic map after exposing the ear to intense noise. By understanding how these changes happen, researchers could develop techniques to bring the map back to normal and relieve tinnitus.
Loud noise is the leading cause of damage to the inner ear. Most patients with noise trauma describe a whistling tinnitus (Nicholas-Puel et al,. 2002). In a large study of tinnitus, avoidance of occupational noise was one of two factors most important in preventing tinnitus (Sindhusake et al. 2003). The other important factor was the rapidity of treating ear infections.
Limit use of earplugs. Earplugs are important to use to protect your hearing when you’re likely to be exposed to loud noises. (Remember, exposure to loud sounds, and noise-induced hearing loss, are common causes of tinnitus, and may make tinnitus worse if you already have the condition.) But otherwise, people with tinnitus are advised not to wear earplugs, including for sleep. Earplugs reduce your ability to hear external noise and can make tinnitus more noticeable.
As an initial test of our treatment, we first conducted a small pilot study to see if there were measurable benefits within 3 to 6 months of using this therapy. While we did not inform participants of whether they would receive a treatment or unaltered music, every participant in fact received a treatment. Participants reported a drop in scores on the Tinnitus Handicap Inventory (THI) within 3 months of using their personalized sound therapy for about 2 hours a day. THI is a psychometrically robust and validated questionnaire that assesses the impact of tinnitus on daily living and the degree of distress suffered by the tinnitus patient. Furthermore, we saw increased benefits after 6 months of treatment use (Figure 1). This data suggested that our treatment may be engaging brain plasticity in a positive manner, thereby gradually reducing tinnitus over time. Armed with this information, we designed a more rigorous trial that is very uncommon among research in tinnitus therapies.
Masking Devices. Similar to the white noise machines listed above, there are now masking devices that can be worn in the ear, just like a hearing aid, that do almost the same thing. They produce low-level white noise that can suppresses your tinnitus symptoms by training your brain to focus on them instead of the ringing in your ears. These are perfect if you can’t always have a white noise machine running near you.
Tinnitus can be triggered by a variety of different causes, and it varies dramatically from person to person. Some of the causes result in permanent tinnitus that may require treatment, while others result in temporary tinnitus that disappears on its own. Tinnitus can arise anywhere along the auditory pathway, from the outer ear through the middle and inner ear to the brain's auditory cortex, where it's thought to be encoded (in a sense, imprinted). One of the most common causes of tinnitus is damage to the hair cells in the cochlea (see "Auditory pathways and tinnitus"). These cells help transform sound waves into nerve signals. If the auditory pathways or circuits in the brain don't receive the signals they're expecting from the cochlea, the brain in effect "turns up the gain" on those pathways in an effort to detect the signal — in much the same way that you turn up the volume on a car radio when you're trying to find a station's signal. The resulting electrical noise takes the form of tinnitus — a sound that is high-pitched if hearing loss is in the high-frequency range and low-pitched if it's in the low-frequency range. This kind of tinnitus resembles phantom limb pain in an amputee — the brain is producing abnormal nerve signals to compensate for missing input.
Technology and portable music devices also contribute to noise pollution, especially in younger people. Keep the volume of your phone, MP3 players or iPod on the lower end when listening to headphones, and don’t play very loud noises for long durations of time. To aid in tinnitus treatment, look out for changes in your ability to hear if you’re frequently exposed to loud noises, limit use of headphones or consider wearing earplugs.
A large, 2014 study of almost 14,000 people found obstructive sleep apnea was linked to significantly higher rates of hearing impairment and hearing loss. Scientists think one reason for this is changes to blood flow to the ear that result in inflammation. (We know that sleep apnea causes changes to circulation and weakens blood flow to some areas of the body, including the brain.) A related factor? People with sleep apnea are at greater risk for high blood pressure, and high blood pressure can exacerbate hearing loss, according to research.

It can be helpful to take notes of the sounds you are regularly or irregularly experience to help your hearing healthcare professional put together the clues to what may be causing it. Be sure to alert your practitioner of any pertinent medical history, medications or excessive noise exposure that could be playing a role in your tinnitus.
For many, tinnitus symptoms come on gradually and eventually go away as the brain and ears adjust. However, for others tinnitus can last for years and cause various complications. A high percentage of people with tinnitus that’s persistent and untreatable go on to also develop anxiety or depression as a result. What types of things can you do to deal with and lower tinnitus symptoms? Tinnitus treatment includes avoiding excessively loud sources of noise pollution, using certain hearing aids, preventing ear infections and avoiding drug use.
In the advance online edition of Nature on January 12, 2011, the researchers reported that the number of neurons tuned to the high frequency had jumped by 79% compared to control rats. The scientist then tested 2 different tones in a second group of rats but stimulated the vagus nerve only for the higher one. The neurons tuned to the higher tone increased by 70%, while those tuned to the lower one decreased in number. This showed that the tone alone wasn’t enough to initiate the change; it had to be accompanied by VNS.
Schecklmann et al (2014) suggested that tinnitus is associated with alterations in motor cortex excitability, by pooling several studies, and reported that there are differences in intracortical inhibition, intra-cortical facilitation, and cortical silent period. We doubt that this means that motor cortex excitability causes tinnitus, but rather we suspect that these findings reflect features of brain organization that may predispose certain persons to develop tinnitus over someone else.
Cognitive Behavior Therapy (CBT) / Hypnotherapy. Another alternative treatment option worth considering is Cognitive Behavior Therapy (CBT), which is often simply referred to as hypnotherapy. The goal of CBT is to help lower any anxiety, anger or depression you are feeling toward your tinnitus, as well as to help retrain your brain to notice the ringing in your ears less. CBT is typically used in conjunction with sound stimulation therapies, like Neuromonics or Transcranial Magnetic Stimulation (TRT). When choosing your therapist, be sure they not only do they have previous experience working with patients with tinnitus, but also have Neural Linguistic Programming (NLP) training.
It’s been found that exposure to very loud noises can contribute to early hearing loss and ear problems. Loud sounds can include those from heavy machinery or construction equipment (such as sledge hammers, chain saws and firearms). Even gun shots, car accidents, or very loud concerts and events can trigger acute tinnitus, although this should go away within a couple days in some cases. (5)

Currently there is no cure for most cases of tinnitus. Depending on the type of tinnitus, symptoms will tend to come and go over time. Stress level, diet, and exposure to noise can worsen tinnitus. Many people find their tinnitus annoying but can learn to adapt without difficulty. It is likely that if you have had tinnitus, you will have it again in the future.
To answer your question about NAC (N-acetyl-L-cysteine), I’ve seen little evidence suggesting that is effective for tinnitus. Instead – though the research is very limited – multiple anecdotal reports describe success with the herb ginkgo biloba. Try taking two tablets of standardized extract of ginkgo three times a day with meals (no more than a total dose of 240 mg a day). Ginkgo may work by increasing blood circulation to the head and neck. Give it at least a two-month trial. You might also explore cranial therapy, a gentle manipulative technique performed by osteopathic physicians. This approach seems to take the pressure or irritation off the auditory nerves. If high blood pressure is responsible for your tinnitus, try to get that under control through diet, exercise, and weight loss or medication if necessary.
Treatment of the underlying primary disorder may help to improve or cure rhythmic tinnitus. For example, the treatment of blood vessel disorders (e.g. dural arteriovenous shunts) can include certain medications or surgery. A surgical procedure known as sinus wall reconstruction can successfully treat pulsatile tinnitus due to sigmoid sinus diverticulum and dehiscence. In fact, most individuals have experienced complete resolution of their tinnitus following this surgery. Surgery may also be necessary for rare cases of pulsatile tinnitus caused by a tumor.
There seems to be a two-way-street relationship between tinnitus and sleep problems. The symptoms of tinnitus can interfere with sleeping well—and poor sleep can make tinnitus more aggravating and difficult to manage effectively. In the same study that found a majority of people with tinnitus had a sleep disorder, the scientists also found that the presence of sleep disorders made tinnitus more disruptive.
There are, however, excellent tools to help patients manage their condition; treatments that reduce the perceived intensity, omnipresence, and burden of tinnitus. These currently available treatments are not “cures” — they neither repair the underlying causes of tinnitus, nor eliminate the tinnitus signal in the brain. Instead, they address the attentional, emotional, and cognitive impact of tinnitus. They help patients live better, more fulfilling, and more productive lives, even if the perception of tinnitus remains.

When TRT was developed in the 1980s by neuroscientist Dr. Pawel Jastreboff (now at Emory University in Atlanta), it was designed to be administered according to a strict protocol. Today, the term TRT is being used to describe modified versions of this therapy, and the variations make accurate assessment of its effectiveness difficult. Individual studies have reported improvements in as many as 80% of patients with high-pitched tinnitus. In a Cochrane review of the one randomized trial that followed Jastreboff's protocol and met the organization's standards, TRT was much more effective in reducing tinnitus severity and disability than a technique called masking (see below).
While there may be a wide range of causes, an important underlying factor for the development of tinnitus is brain plasticity.5,7 This property allows the brain to change and adapt, and it is essential to how we learn. Unfortunately, in some cases, such as with hearing loss, the auditory part of the brain may be altered as brain plasticity tries to compensate for the abnormal auditory inputs. This response leads to changes in brain activity in the auditory system (e.g., the auditory cortex) that can create a phantom percept: tinnitus. As such, while tinnitus may begin a problem at the auditory periphery, it persists because of changes throughout the auditory system. Treating tinnitus may require addressing both the initiator (e.g., hearing loss) and the driver (changes in the auditory brain).
Even with all of these associated conditions and causes, some people develop tinnitus for no obvious reason. Most of the time, tinnitus isn’t a sign of a serious health problem, although if it’s loud or doesn’t go away, it can cause fatigue, depression, anxiety, and problems with memory and concentration. For some, tinnitus can be a source of real mental and emotional anguish.
Most cases of tinnitus are unfortunately thought to be difficult to treat, and sometimes severe tinnitus cannot be treated at all when permanent and irreversible damage to the ears or nerves has occurred. That being said, many patients find natural tinnitus treatment methods and coping strategies to be very helpful in allowing them to adjust to the changes that tinnitus brings. According to the American Tinnitus Association, most cases of tinnitus are caused by hearing loss. Occasionally though, tinnitus is caused by an irritation to the auditory system. Tinnitus can sometimes be a symptom of a problem with the temporomandibular joint (TMJ). If your tinnitus is caused by TMJ, then a dental procedure or realignment of your bite may alleviate the problem.
Although there’s no proven cure for tinnitus, there are treatments that help make it easier to ignore. For example, you can wear devices in your ear(s) that produce soothing therapeutic noises to shift your focus away from the tinnitus. Other devices produce constant, soft noise to mask the tinnitus.
